Lack of motor oil pressure or other connecting rod bearing failure usually, or over-revving and resultant stress cracking of the rod itself, winds up breaking the rod into two or more pieces while the motor is running and many times these pieces get thrown through the side of the block. Nasty. Oil pressure is lost because of lack of oil, pump failure, or excessive rod and main bearing clearances or other excessive "leak" of oil pressure.
