Clutch cable removal

OK, my cable is frayed pretty bad.  How do I get the cable off of the tranny bracket?  And when ordering a new clutch cable, do I need any other parts?  Is there supposed to be a rubber grommet thru the firewall, cause mine looks like hard plastic. (Oh yeah, it's been converted to a manual adjusting clutch cable by the PO.)
