Flange cap seals

sounds like he drilled a hole in the center of it to pop it back off. probably forgot the circlip or something. or he didnt replace the cap at all on that side and reused the old one.

  I replaced both flange seals on the FN tranny. the outer big ones. I also replaced both the caps/seals that reside inside the flanges. But the one on the drivers side had a hole in it,it was machined on the joint side . My mechanic had replaced that cap 20K miles ago. Is it possible he put the wrong one? I thought there was only "one size fits all",
  What would cause that to happen ? improper installation?
