Yup.

Spend the 40$ and get a new disk. Be sure to clean the surfaces of the 
flywheel and pp thoroughly with brake cleaner before you put the new disk 
in.

Possibly an inner CV joint. How are the boots? Is there any circumference 
play in the joint with respect to the axle?

What is the condition of your exhaust system? Is it factory? Aftermarket? 
Aree your hangers good? Is there a rock in your cat guard? Do you have the 
anti-rattle clips on your brake pads?
