Yes, replace both.  I had the same exact problem a while ago and I replaced 
both the pos, and neg cables and it cleared the problem right up.  The way I 
figured out that was the problem was whenever I touched and moved around 
these cables the problem would clear up for a day or two.  It may also be 
helpful to add another ground wire from the alternator.
