Shifting into 5th is nervous; Update

Ok I've been busy and didn't have time to work on my car during the past
week, but now that I have freed up some time I checked my tranny fluids
and everything is ok.  But now there is a clanging sound coming from the
tranny.  When the car is idling everything seems ok, but when I shift
into gear (with some difficulty) there is this clanging sound that can
be heard when I shift into gear so far I went up to 3rd gear and that
clanging sound can still be heard, after that I parked the car.  Does
anybody out there would know what is wrong.
